LIABILITY FORM & IMAGE RELEASE
LIABILITY
This agreement releases (MISSION BREAKERS LLC) from all liability relating to injuries OR sickness, including COVID 19, that may occur during any physical activity or any performer/equipment. By signing this agreement, I agree to hold (MISSION BREAKERS LLC) entirely free from any liability, including financial responsibility for injuries OR sickness incurred, regardless of whether they are caused by negligence. I also acknowledge the risks involved in all physical activities practiced inside the STUDIO of MISSION BREAKERS. These include but are not limited to: Break dancing, Mixed Martial Arts, and Bodybuilding. I swear that I am participating voluntarily and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries OR sickness while engaging in these activities. By signing below forfeit all rights to bring suit against (MISSION BREAKERS LLC) for any reason. In return, I will receive eligibility to use the MISSION BREAKERS facility. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally.
IMAGE RELEASE
I hereby grant and authorize (MISSION BREAKERS LLC) the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me by to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing. I understand and agree that these materials shall become the property of and will not be returned. I hereby hold harmless, and release from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.
